Due to recent modifications in the guidelines for CME certification, meals and libations cannot be provided for attendee’s family or other guests. As a result, reasonably priced meal tickets will be made available for the two dinners and the Saturday luncheon. Additionally, a “cash-bar” will be available at the dinners.

Please fill out the brief questionnaire below so that we may plan the various functions based on attendance. In order to defray the costs of meals for guests and CME, please mail a check to the address below or pay at registration.
